Our Policies:
-
Functionally You currently provides services to clients located in Ohio and select U.S. states where telehealth services and provider-ordered lab testing are permitted.
At this time, services are available in the following states: Ohio, Arizona, Colorado, Delaware, Kansas, Maryland, Massachusetts, Nebraska, New Mexico, Oregon, Rhode Island, Utah, Vermont, and Washington.
Clients must be physically located in an eligible state at the time of service and at the time lab orders are placed.
Service availability may change over time as we expand into additional states.
-
By participating in services with Functionally You, you consent to receiving care via telehealth. Telehealth involves the use of electronic communications to enable healthcare services when you and your provider are not in the same physical location.
You understand that telehealth may have limitations compared to in-person care and that you have the right to request in-person services when available.
You also acknowledge responsibility for ensuring you are located in an eligible state at the time of service.
-
All services are self-pay and due at the time of booking.
Due to the time, preparation, and clinical planning involved in your care, all program purchases are non-refundable.
By enrolling in a program, you are committing to the full duration of care (typically 3–6 months), allowing adequate time for meaningful results and appropriate adjustments.
If services cannot be provided due to location restrictions or other eligibility factors, a refund will be issued minus any applicable administrative or processing fees. Once lab orders have been placed or services have been rendered, refunds are not available.
-
We understand that schedules may change. If you need to cancel or reschedule your appointment, please do so at least 24 hours in advance.
Appointments canceled with less than 24 hours’ notice or missed without notice may be subject to a cancellation fee or forfeiture of the session.
Late arrivals may result in a shortened session to respect scheduled time commitments.
-
Functionally You offers secure messaging through the client portal to support your care between visits.
Messaging is intended for brief, non-urgent questions or clarifications. Response times may vary, but messages are typically addressed within 1–2 business days.
The level of messaging access may vary depending on your selected program or level of support.
Messaging is not intended for urgent or emergency concerns. If you are experiencing a medical emergency, please call 911 or seek immediate care.
-
All prescription decisions are made at the sole discretion of the provider and are based on clinical judgment, safety, and medical appropriateness.
Enrollment in a program or consultation does not guarantee that medications will be prescribed. All recommendations are individualized and based on your health history, laboratory findings, and overall clinical assessment.
-
Lab testing is included as part of the initial consultation experience. Clients are responsible for completing lab work in a timely manner. Results are used to guide personalized recommendations and next steps in care.
Additional or specialty testing may be recommended based on individual needs.
-
Individual results may vary. Functionally You does not guarantee specific outcomes, as results depend on many factors including adherence to recommendations, underlying health conditions, and individual response to care.
-
Functionally You is not an emergency service.
If you are experiencing a medical emergency, please call 911 or go to your nearest emergency room.
Information provided through Functionally You is for educational purposes and is not intended to replace individualized medical advice, diagnosis, or treatment. All care and recommendations are based on your unique clinical assessment and should be followed in consultation with your provider.
